We deliver a 24/7 Careline and Response Service to 3000 customers, and Support Services to 2000 customer to help them maintain independent living in their own home, and we have an exciting opportunity to further enhance our position as a leading provider of these services in Derbyshire.
You will provide support to vulnerable and older adults in accordance with the principles of the dignity and respect agenda, to assist them to sustain independent living in their own homes, identifying support needs and risk through completing person centred assessments, and making referrals to appropriate agencies or services to ensure their needs are identified and assessed.
You must have excellent communication skills, including the ability to communicate sensitively and effectively with vulnerable people of all ages and cognitive ability, and an understanding of the needs of older and vulnerable people.
You must have a current Drivers Licence and access to a vehicle during working hours, and be able to work on your own initiative, with minimal supervision.
You should ideally have experience of using computer systems and must have the ability to accurately input and extract information from IT systems, completing accurate, clear and concise reports.
Due to this post having access to children and/or vulnerable adults, candidates will be required to undertake a Disclosure and Barring Service check. The possession of a criminal record will not necessarily prevent an applicant from obtaining this post, as all cases are judged individually according to the nature of the role and information provided.
You will also be required to subscribe to the DBS online Update Service.
